- Club name and town/county
- Your role (chairperson, secretary, coach or authorised representative)
- Approximate teams / players in the club
- Age groups (for example U7–U16 or adult teams)
- Club website or social page (if available)
- Products of interest (optional)
- Badge permission – confirm that you are authorised to discuss the club badge and merchandise with Player Prep
We will review your enquiry, explain the setup process and agree the club products and reward terms before publishing a shop. Please do not send sensitive player information or children's personal details in this form.
